Anorectal Malignant Melanoma Masquerading as a Neuroendocrine Tumour on Initial Histopathology: A Diagnostic Pitfall in a Pedunculated Anal Mass

Om Prakash Bhandari, MS, MRSH (London), FAIS

A 70-year-old woman's pedunculated anorectal mass, initially read on H&E as a neuroendocrine tumour, was revised to anorectal malignant melanoma after a broadened immunohistochemistry panel (S-100, SOX10, MelanA, HMB45 positive). A case-based teaching review for trainees on why a negative first-line IHC panel should prompt an expanded, full-lineage panel rather than diagnostic closure. Peer-reviewed and cleared for publication by Dr. Sadhana Kothari, Associate Professor of Pathology, GCS Medical College, Ahmedabad.

Implanted Appendicolith at the Laparoscopic Port Site Mimicking an Enterocutaneous Fistula

N. K. Malpani, MS, FAIS

A rare case of a spontaneously implanted appendicolith at the umbilical retrieval port following laparoscopic appendicectomy, presenting as a discharging sinus that mimicked an enterocutaneous fistula. Three variably sized stones were recovered from the wound, with spontaneous healing thereafter — possibly the first reported case of its kind in the world literature. Peer-reviewed and cleared for publication by Dr. Sandeep Purohit, MCh (Urology).

O. P. Bhandari, MS, MRSH (London), FAIS

An 8-year-old child's complete unilateral cleft lip repair at a district-level Rajasthan operation theatre in 2004, documenting the Millard rotation-advancement technique alongside historical anaesthetic practice — ether anaesthesia, a primitive pulse oximeter, and manual pulse monitoring — in a resource-limited setting. Surgical technique peer-reviewed by Dr. M.P. Agrawal; anaesthesia section peer-reviewed by Dr. Anand Derashri.

O. P. Bhandari, MS  ·  P. Bajaj  ·  S. R. Somani  ·  Y. C. Vora  ·  Ravindra Nath Tagore Medical College, Udaipur

A historical paper documenting the independent derivation of percutaneous venous cannulation — using a BT set needle as introducer and polyethylene venesection tubing as catheter — in the tetanus ward of RNT Medical College, Udaipur (1977–79), without prior knowledge of the Seldinger technique.
